Preparing For Your Visit

My Family ENT aims to make preparing for your visit as easy as possible, and for most patients, that means knowing what to expect. Whether you want more details on what your initial appointment will consist of, or a checklist of things you should bring, click and enjoy the peace-of-mind that comes with coming prepared.

Please bring your current insurance card and photo ID. Non-parent guardians must bring a current Power Of Attorney. We accept credit cards and cash for payment, but we do not accept checks. Also, please complete the Patient RegistrationConsent, Notice of Privacy Practices and New Patient History forms.

Adult patients, it is also helpful if you bring a list of your medical conditions, medications that you are taking, and any relevant records from other treating physicians.
